What is a Community Navigator?
The aim of a Community Navigator is to help individuals to take steps to improve their health and wellbeing.
A person’s health can quite often be improved without medical involvement. A navigator will work with individuals who need additional support with non-medical issues such as, isolation, loneliness and loss of motivation and confidence. They will firstly assess a person’s needs and then guide them to local community organisations and voluntary groups for advice and support.
How can I access the service?
You can ask the GP, Practice Nurse or Healthcare Assistant to refer you to the service or you can refer yourself by contacting Conwy Single Point of Access
