Medication review

If you’re taking medication on a regular basis you’ll need to attend for a medication review.  This will be annually or six monthly, depending on your medication.

The review is an opportunity for us to check your health and to make sure that your medication is working and that you’re on the correct dose etc.

The date that your next review is due is printed on your repeat medication slip, please arrange an appointment with the Health Care Assistant on or around this date.  It’s very important that you attend.  If you repeatedly fail to attend for a review, you will be given a 7, rather than 28 day’s supply of your medication until you attend.  This being to safeguard you.

The Health Care Assistant will do a health check and take your bloods and the results reviewed by the GP.  You should contact reception to ask for your results after three days.  If all is well no further action is needed until your next review.  If there are any issues, you will be informed when you call for your results and an appointment will be made for you to have a telephone consultation with the GP.
