A Doctor’s Note or Sick Note is now called a ‘Fit Note‘

Illness for less than 7 days

If you are unwell and absent from work for 7 days or less, you do not need a doctor’s note. In these instances, you can ‘self certify’ your illness.

Your employer might have a procedure or form for you to complete or you can use the SC2 Self-certificate Form.

Illness longer than 7 days

If you are unwell and away from work for 7 days or more, you will need a Fit Note.

You do not need to make an appointment with the doctor for this. Please tell receptionist the reason and dates of absence and a request will be made to the Doctor for your Fit Note. You can also email us your request.

Please allow two working days for your request to be processed.

Fit Notes can be collected from reception or sent electronically via email or SMS.

You do not need a Fit Note to return to work.

Self-Certification sick/fit note for less than 7 days

If you have been ill for 7 days or less you don’t need to see a doctor. You can complete a Self-Certification form yourself. However some employers insist on a doctor’s note regardless. A doctor’s note under these circumstances may involve a charge.

Many employers have their own self-certification forms. If your employer doesn’t have its own form you can download the Self Certification Form. Please print it, fill it in and hand it in to your employer. You do not need to see a doctor.

Doctor’s sick/fit note for more than 7 days

If you have been ill for more than 7 days you will need a doctor’s certificate. These certificates are called ‘Fitness To Work’ Certificates. The doctor may call you or arrange for an appointment. If you have already been seen by your doctor about this illness your certificate will be sent to you within a week.
